Set four years after No Way Home, the story follows Peter Parker living in total state of being unknown or unrecognized by others. Doctor Strange’s spell erased every memory of his existence from the world, including his closest friends MJ and Ned. He now operates as a full-time Spider‑Man, protecting New York while watching the people he loves move on without ever knowing who he is.

Everyone on earth forgets that Peter Parker is Spiderman. 
Spiderman saves the life of Francis "Frank" Castle and he entrusted him to look after MJ but it turns out that the person needed the help most is him.

Peter Parker is undergoing changes, his senses are getting stronger as he aged, just like an arachnid molts or shed their skin, about 5-10 times, to grow from a baby into an adult.

The most fascinating about this movie is that Peter Parker created his own A.I. support computer E.V.: His self-made AI companion, voiced by Naomi Watts. An addition to his A.I. Karen, the A.I. from his spiderman suit given to him by Tony Stark (voiced by Jennifer Connelly); but in this movie Spiderman: Brand New Day we never hear the voice of Karen maybe it is because of the spell that even Stark Enterprises forgot the existence of Spiderman.

The weight of this solitude triggers an unexpected, uncontrollable evolution in his powers—including organic webbing and heightened abilities he struggles to contain. At the same time, a mysterious, nearly invisible threat emerges: Jean Grey, a powerful telepath driven by grief and corruption, who threatens the city and those Peter still cares for. Jean Grey knows spiderman's identity.

Aunt May appears in the movie through flashbacks and a mind-scape sequence, her words help calm Jean Grey.

With help from allies like Bruce Banner / Hulk, Frank Castle / The Punisher, and unexpected connections, Peter faces his greatest challenge yet: not just saving the city, but reconciling the two halves of himself. Accepting that he is both Peter Parker and Spider‑Man, and learning that strength means letting people in, even if they don’t remember him.

You will be disappointed if you expect MJ and Peter Parker to get back together.
